This contact form script will create a form like the one below. You can edit it as you like.

To place this form on your website, follow the 3 steps below:

Step 1: Copy and paste the e-mail form code below into a web page and save it as "contact.php" page.

Step 2: Copy and paste the "sendemail.php" code below into another file and save the the file as sendemail.php.
